How it works
Activates AMP-activated protein kinase (AMPK), the body's metabolic master switch. Improves insulin sensitivity, reduces hepatic gluconeogenesis (liver glucose production), inhibits intestinal glucose absorption, upregulates LDL receptors in the liver (independent of statin pathways), and beneficially modulates gut microbiota composition. Also inhibits CYP3A4 and P-glycoprotein, which causes significant drug interactions.
Evidence-based uses
Berberine HCl is the standard form. Start with 500mg once daily for 1-2 weeks to minimise GI side effects. Most clinical trials use 1500mg/day divided into 3 doses. Effects on HbA1c comparable to metformin in head-to-head trials.
Clinical Evidence
Berberine 500mg 3x/day showed significant reductions in HbA1c, fasting blood glucose, and triglycerides, comparable to metformin.
Analysis of 27 RCTs (2,569 patients) confirmed berberine significantly reduces fasting blood glucose, HbA1c, total cholesterol, LDL, and triglycerides with a safety profile comparable to conventional treatments.
Largest meta-analysis to date (41 RCTs, 4,838 patients). Berberine reduced total cholesterol by 17.4 mg/dL, LDL by 15.0 mg/dL, and triglycerides by 18.7 mg/dL while raising HDL by 2.0 mg/dL.
Confirmed berberine significantly improves all five metabolic syndrome components: blood pressure, waist circumference, fasting glucose, triglycerides, and HDL cholesterol.

Clinical Evidence

Berberine selectively inhibited pathogenic bacteria while increasing beneficial SCFA-producing species including Butyricimonas, Coprococcus, and Ruminococcus. Also boosted Lactobacillus and Akkermansia populations.
Herbal therapy containing berberine showed equivalent SIBO eradication rates to rifaximin (46% vs 34%, p=0.24), with berberine-containing herbs actually showing a numerically higher response rate.
Berberine may support energy through improved glucose metabolism and AMPK activation. Primary evidence is for blood sugar management, not direct energy improvement.
Safety
Both lower blood sugar. Combined use may cause dangerous hypoglycemia. Do not combine without medical supervision.
Berberine inhibits CYP3A4, significantly increasing cyclosporine blood levels. Avoid combination.
Berberine inhibits CYP3A4 which metabolises many statins. May increase statin levels and side effects.
Berberine inhibits CYP3A4 and P-glycoprotein, significantly increasing tacrolimus blood levels. Narrow therapeutic window makes this combination dangerous without close monitoring.
Berberine may lower blood pressure. Combined with antihypertensives, this could cause hypotension. Monitor blood pressure if combining.
Berberine lowers blood sugar through multiple mechanisms. Combined with insulin or sulfonylureas, the risk of dangerous hypoglycemia is significantly increased. Requires dose adjustment and close monitoring.
Berberine crosses the placenta and is linked to neonatal jaundice and hyperbilirubinemia. Also passes into breast milk. Contraindicated during pregnancy and breastfeeding.
Berberine inhibits CYP2D6, which metabolises many antidepressants. May increase blood levels of fluoxetine, paroxetine, venlafaxine and others.
Berberine may affect CYP2C9, potentially altering warfarin metabolism. Monitor INR closely.
